General Documentary Requirements*

For Domestic Entities:

1. One (1) valid photo-bearing government-issued identification card of the following:

  1. Authorized Signatories
  2. Ultimate Beneficial Owners (UBOs)
  3. Corporate Secretary
  4. Nominee and Alternate Nominee (for one person corporation)

2. Valid and latest Certificate of Registration issued by DTI, SEC, BSP, or Appropriate Government Agency, as applicable.

3. Articles of Incorporation/Partnership or Association and By-Laws.

4. Notarized Board or Partnership Resolution duly certified by the Corporate/Partner's Secretary authorizing to sign on behalf of the entity.

5. Latest General Information Sheet or Notarized Appointment of Officers (for one person corporation).

For Foreign Corporations:

Similar documents issued by a foreign country where the entity is registered and duly authenticated by the Philippine Consulate or Philippine Embassy where the entity is domiciled or bearing an apostille from a Competent Authority where the public documents were issued.
